Referring an adult to be admitted to Ascension Brighton Center for Recovery
Physicians and authorized caregivers can refer an adult 18 years and older to Ascension Brighton Center for Recovery for addiction care treatment by completing our Secure Online Admission Form or calling 1-877-9SOBER1 (877-976-2371). What to expect
Once we’ve received your admissions form, we will review it within the next 24 hours and go over your insurance benefits. Our team will contact you to schedule a clinical assessment at Ascension Brighton Center for Recovery. Our holistic approach means we start by listening to understand you and your health needs. To be admitted, you must be an adult 18 years and older and not have a current medical or psychiatric health condition that requires inpatient (hospital) care. Our comprehensive assessment includes:
- A physical, mental and behavioral, and substance use history screening by a certified clinician specializing in addiction care
- Lab testing for substances, and screening for COVID-19 and influenza
- Blood work to evaluate general health
- Meeting with a medical staff member to review any past or current medical or psychiatric conditions

Our doctor may request a copy of your medical records. Please print and complete the Ascension Michigan Release of Medical Records Authorization form. This form can be used to release patient-identifiable health information to anyone a patient authorizes in writing to receive such information.
