Community Health Needs Assessments

Every three years, our hospitals are required to perform community health needs assessments (CHNAs) to evaluate the overall health status of the communities we serve. Input from persons who represent the broad interest of these communities is taken into account during the assessment. The community-driven approach supports our organization in planning and offering programs to address the health needs of a community, with special attention to persons who are under served and vulnerable. In collaboration with our local community health partners, we are pleased to present the following CHNA reports and implementation strategies.
