Ascension Medical Group Illinois - Geriatrics Elk Grove Village is a "C3" clinic designed for older patients who are managing multiple conditions at once. These include but are not limited to:

The C3 clinic helps to coordinate care for you or your loved one's specific needs. Our multispecialty care team includes a board-certified doctor specializing in family medicine and geriatrics, a case manager/social worker, a clinical pharmacist and a dedicated medical assistant. We work together to:
- Create a personalized care plan for your specific needs and goals
- Provide appointments where you can ask questions and talk about your health concerns
- Deliver additional support such as follow-up phone calls between doctor visits
