To receive a copy of your medical records from one of our clinic locations, please contact the clinic directly for further instructions.

Hospital Medical Records Requests

To receive a copy of your medical record, an authorization to release medical information form must be signed and dated by the patient or the patient’s legal guardian. Authorization forms are available in either the Ascension Seton Health Information Management department, the Dell Children's Medical Center Health Information Management department, or downloaded from this page.

You have the opportunity to request traditional paper records or to receive your records electronically. Please mail your completed form to the Release of Information Department at the site where you received care or return it in person. If you are returning the form in person, you must have proper identification. Due to privacy and security concerns, we are unable to accept faxed or emailed requests.

If you have questions about an adult request, the Ascension Seton Health Information Management department, call 512-324-1490. Our hours are Monday - Friday from 8:30 a.m. to 5 p.m., with a lunch closure from 12 to 12:30 p.m.

If you have questions about a pediatric request, call 512-324-7261. Our hours are Monday - Friday from 8:30 a.m. to 5 p.m., with a lunch closure from 12 to 12:30 p.m. If you are a patient of Dell Children’s Medical Group at Strictly Pediatrics or other Central Texas locations, please contact: 512-596-0292.

Please allow two weeks to receive a copy of your requested medical records; please note that some records are not complete until 30 days after discharge as they are awaiting transcribed reports from healthcare providers. In addition, we are unable to release radiology images with your medical record.

Requestors may be charged a fee to cover the expense to provide copies of health information; the fees are set by the Texas Health and Safety Code and are updated annually.
