At Ascension Seton and Dell Children’s Medical Center, we make accessing you or your child’s health information convenient with our patient portal.

You can view information from your visit at Ascension Seton or Dell Children’s Medical Center through our patient portal. There are separate portals for hospital visits and medical office visits.

Your patient portal information includes a summary of your care at the hospital or your doctor’s office, including:

  • Active medication list, allergies, vaccinations and procedures
  • Future appointments
  • Hospital discharge instructions
  • Lab and vital sign results

To enroll for the medical office portal (Seton iConnect), please ask your Ascension Seton or Dell Children’s Medical Group doctor when you are in their office.

To enroll in the My Seton Health portal, which contains your hospital care information, please fill out the appropriate form below and bring it, along with a government-issued ID, to any Ascension Seton or Dell Children’s location. You will then receive an email to complete your registration.

Patient portal adult form (Form A)

  • Patients 18 years or older
  • Access to their own portal
  • Form signed or verbal consent documented; scan form
  • Government issued ID required for verification purposes

Patient proxy adult form (Form B)

  • Patients 18 years or older, giving access to a parent or guardian
  • Signed by patient and proxy and scanned
  • Government issued ID required for verification purposes
  • Use Form D to revoke access

Patient Portal Minor Proxy Form (Form C)

  • Patients 0-12 years old with a parent or guardian requesting access
  • Form signed and scanned
  • Government issued ID required for verification purposes

Patient Portal Minor and Proxy Form (Form D)

  • Patients 13-17 years old with a parent or guardian requesting access
  • Signed by patient and proxy and scanned
  • Government issued ID required for verification purposes
