As a patient, you have the right to access your medical records. Here you'll find instructions and a convenient form to help us process your request.

If you would like to request a copy of your medical records, please contact the Health Information Management department at 607-798-5970 and choose option #3.

  • You may request your medical records in writing to either our Health Information Management Department or the Practice Manager of your primary care office. For privacy reasons, patient identification must be verified before any information can be released.
  • Please complete an Authorization for the Release of Health Information form and give it to your primary care provider or mail the form to:
