Follow-up care after your hospital stay
We specialize in post-hospital follow-up and complex chronic care. The goal of the Bridge Clinic is to provide you access with one of our providers after being discharged from the hospital. We will work together with your primary care physician and/or specialist physician(s) to help coordinate your continued follow-up care and transition you back to their care as soon as possible. If needed, our staff is also here to help you find a primary care physician.
The Bridge Clinic services include:
- Quick and easy access with a physician referral
- Appointments lasting up to 1 hour to allow plenty of time to ensure your health is progressing and for you to ask questions
- Medication reconciliation
- Promote patient safety when transitioning from hospital to home
- Necessary laboratory testing with review of the results
- Help in obtaining additional healthcare services
- Assistance with scheduling needed follow-up appointments or finding a primary care physician and/or specialists
