In southeastern Wisconsin, Sacred Heart Rehabilitation is conveniently located on the Ascension Columbia-St. Mary’s Hospital Milwaukee campus on North Lake Drive.  We personalize each specialty program for your needs:

Sacred Heart Rehabilitation Hospital is a recognized Medicare and Medicaid inpatient rehabilitation facility (IRF). Medical studies report IRFs provide a higher level of functional improvement in a shorter time period compared to rehab in a nursing home environment. Our team approach means greater involvement from doctors specializing in brain, heart, orthopedic, spine physical rehabilitation medicine. Ascension’s highly trained, compassionate nurses, respiratory therapists, counselors, and rehabilitation therapists (PT, OT, and speech) use innovative therapies to enhance recovery.  
Our patients have access to 24/7 medical imaging, lab and surgical services, should the need arise.
Since 1965, Sacred Heart Rehabilitation Hospital has been a nationally recognized post-acute care facility, accredited by the Joint Commission on the Accreditation of Hospitals and the Commission on the Accreditation of Rehabilitation Facilities (CARF). 

Personalizing your Rehabilitation Therapies

Rehabilitation is a group effort by the patient, family member and our rehabilitation team. To ensure consistent, high-quality care and services, we assemble a doctor led rehab team that remains with the patient throughout his or her stay. Team members may include:

  • Physical Medicine and Rehabilitation
  • Social worker/case manager
  • Certified rehabilitation nurse
  • Occupational therapist
  • Physical therapist
  • Speech and language therapist
  • Respiratory therapist
  • Recreational therapist
  • Rehabilitation psychologist

Additional team members from other specialties or a vocational counselor may be added as needed.

Four-legged Healers at Columbia-St. Mary’s Sacred Heart 

There’s a bond between humans and dogs that defies understanding. Animal-assisted therapy helps patients attain functional goals (such as reaching, stroking, speaking). In return these specially trained dogs are patient and comforting. 
At Ascension we prepare you for transitioning to your home, work and community activities. Home safety evaluations are done and community outing to practice skills.
