Telehealth Maternity Care Program in Jacksonville
If you're pregnant or recently had a baby and need extra support, Ascension St. Vincent's Southside partners with the Florida Department of Health to provide personalized pregnancy and postpartum care. Eligible participants are paired with a dedicated patient navigator who coordinates care, answers questions and connects families with education and community resources throughout pregnancy and for up to one year after delivery at no cost.

What is the Telehealth Maternity Care Program?
The Telehealth Maternity Care Program combines virtual support, patient navigation and connections to community resources to help improve access to prenatal and postpartum care.
Participants receive personalized care throughout pregnancy, help scheduling appointments and referrals and support accessing services that promote maternal and infant health.
Personalized care with a patient navigator
A patient navigator works with you throughout your pregnancy and after delivery to help make your care easier to manage. Your patient navigator can support you by:
- Answering questions and providing education throughout pregnancy
- Coordinating prenatal, postpartum, referral and follow-up care
- Connecting you with medical providers and community resources
- Helping you access appropriate medical devices and ongoing support
Pregnancy support services and resources
Depending on your needs, your navigator may also connect you with:
- Baby care products and community resources
- Car seat education and installation
- Home visitation and family support services
- Infant CPR education
- Lactation, nutrition and mental well-being support
Remote monitoring support
Remote monitoring allows your care team to stay connected and identify concerns early without requiring every visit to be in person. When appropriate, participants may receive remote monitoring devices such as:
- Blood pressure monitor
- Digital scale
- Doppler
- Glucose monitor
- Thermometer
Telehealth equipment may also be available when needed.

This program may be a good option if you:
- Are pregnant or recently gave birth
- Have transportation or other barriers to care
- Started prenatal care later than recommended
- Need additional pregnancy or postpartum support
- Would benefit from care coordination and community resources
