What is continuing care?
If you need additional support outside of a hospital, our care teams can provide compassionate medical care and therapy services that help patients recover from or manage an illness or injury. Your care team may include internal medicine specialists, nurses, wound care specialists, respiratory therapists, physical, occupational and speech therapists, registered dietitians, and more.
