To have your medical records sent to your new Ascension provider, you’ll need to establish care in person at your Ascension provider’s office and complete a Release of Information (ROI) form. The office will use this form to request your records from your previous provider.
This process must be completed in person at your Ascension provider’s office and cannot be completed online or over the phone.

Yes. Ascension Seton and Dell Children’s offer virtual care options so you can see a provider from home — ideal for non-emergency concerns, follow-ups or when you're still getting settled in Central Texas. Virtual visits are available for both new and existing patients and can be found using the “video visits” filter when searching for a provider.
