The Ascension Transfer Center at Ascension St. Vincent and Peyton Manning Children’s Hospitals provides a centralized point of access for interfacility transfers. With just one phone call, you can transfer your emergent* patient quickly and prevent delays in care. 

*Emergent patients will be evaluated to ensure they meet admission criteria for an interfacility transfer. We can help you determine appropriate transfers.

How Ascension Transfer Center works

After placing your call, our experienced transfer coordinator nurse will handle the complete transfer process, including:

  • Consulting with the accepting physician
  • Arranging for a patient bed
  • Communicating with the receiving RN for report
  • Assisting with patient transport for helicopter and ambulance transfers
  • All other transfer needs

Our goal is to ensure the continuity of care for your patient. If there is a delay, we will keep you informed every step of the way.

  • Service needed
  • Patient name
  • Basic patient condition

In preparation for your patient’s air or ground transport

Before arrival, our team needs the following information about your patient:

  • Copy of complete patient chart
  • Hospital — patient face sheets — two copies
  • Physician transport summary
  • Copy of X-ray or other imaging on CD or duplicates, if not available on cloud
  • One full page of patient labels
  • Copy of EMTALA form
  • Additional newborn documents and information, including maternal prenatal, antepartum, and labor and delivery records
