Specialty healthcare for seniors

At Ascension Medical Group St. Vincent - Indianapolis Center For Healthy Aging, our team of experienced geriatricians provide personalized care during your geriatric consultations. Your care team evaluates and discusses with you the 4Ms to create a care plan that’s right for you:

What Matters: your health outcome goals and care preferences, including but not limited to, end-of-life choices.

Mentation:  memory, dementia, and mood concerns. Formal memory testing may be recommended.

Medications: how your medications can affect the 4Ms. 

Mobility: how to maintain function and do what matters most.

You can be referred by your doctor , a nurse practitioner, family member, friend, caregiver, or you can make a self-referral. Most major insurances cover this service as a specialist physician visit, please check with your insurance first if you have questions. 

Our team will take the time to get to know you, your family, and your caregivers.

Center for Healthy Aging services

Your care team provides services for your health and well-being, including

  • Caregiver resources
  • Fall Prevention Clinic
  • Nutrition education 
  • Senior counseling 
  • Senior neuropsychological testing for memory loss and cognition changes (including dementia)

What to expect at your appointment

Geriatric Consultations consist of a 1 hour long appointment for evaluation of specific geriatric concerns. Such concerns may include memory loss or other change in mental ability, falls, polypharmacy (taking many medications with possible interactions), frailty, and/or having multiple health issues. Family and/or caregivers are necessary for a complete appointment and assessment. Additional testing may be ordered, including blood work and/or neuropsychological testing. Once testing results are back the Geriatric Conference will be scheduled with the patient and family. 

The Ascension Medical Group St. Vincent - Indianapolis Center For Healthy Aging provides an annual conference for healthcare professionals and a Dementia Education Series for caregivers.
