About Us

The Providence My Health GPS program in Washington, DC provides services to individuals on Medicaid.

In coordination with the District of Columbia’s (District) Department of Health Care Finance (DHCF), the Providence Health Center has a convenient My Health GPS office in our community.  Medicaid beneficiaries with multiple chronic conditions coordinates care with the primary care doctors and care teams at Providence.  My Health GPS is a central point for coordinating your:

  • Primary and acute care
  • Behavioral and mental health services
  • Long-term health services, including skilled nursing care and rehabilitation therapies

This program helps reduce avoidable and preventable hospital admissions and ER visits. My Health GPS program will deliver care coordination services to beneficiaries with multiple chronic conditions, enrolled in either Fee-For-Service or Managed Care.
