Follow-up care after your hospital stay
We specialize in post hospital follow-up and complex chronic care. The goal of Post Discharge Clinic is to provide you access with one of our providers after being discharged from the hospital. We will work together with your primary care doctor or specialist to help coordinate your continued follow-up care and transition you back to their care as soon as possible. If needed, our care team is also here to help you find a primary care physician.
The Post Discharge Clinic services include:
- Appointments lasting up to 1.5 hours to allow plenty of time to ensure your health is progressing and for you to ask questions.
- Education and management of your current condition and other chronic illnesses.
- Information on your medications prescribed at discharge.
- Prescription ordering.
- Pharmacy consultation where appropriate.
- Needed lab work with review of the results.
- Help in obtaining additional healthcare services.
- Assistance with scheduling needed follow-up appointments with your primary care doctor or specialists.
- Assistance with finding a primary care doctor, if needed.
