Working in partnership with Health Care for the Homeless of Milwaukee and the St. Benedict the Moor Ministries, Ascension St. Ben’s Clinic offers personalized, compassionate primary care to people who do not have traditional access to health care.
Ascension St. Ben’s Clinic is located on the site of one of the city’s largest meal programs, Loaves and Fishes, sponsored by St. Benedict the Moor Ministries.
Each visit begins with listening, offering hope and compassion along with health care. This allows our caregivers to treat the immediate healthcare concern and determine if referrals are need for additional care beyond our clinic.
Our primary care doctors and family medicine nurse practitioners provide a wide range of services from preventive health screenings and immunizations to treating acute illnesses.

We are part of the Ascension system, which is dedicated to caring for the whole person – body, mind and spirit. This enables our care givers to quickly arrange for further healthcare services that may be needed. We can also arrange for outreach services and make referrals to other resources in the community, as needed.
Ascension St. Ben’s Mobile Health Clinic
The Ascension St. Ben’s Mobile Health Clinic, in partnership with Outreach Community Health Centers, brings compassionate, high-quality healthcare directly to Milwaukee neighborhoods. Staffed by Ascension Wisconsin nurses, the mobile clinic provides preventive and acute care for adults, along with care coordination and referrals to additional services.
Services include:
- Treatment for common illnesses and minor health concerns
- Chronic disease management, including high blood pressure and diabetes
- Medication management
- Vaccinations
- Care navigation and referrals for behavioral health, prenatal care, specialty care, mammography, and dental services
- Connections to community resources and social services
