Our Ascension St. John Transfer Center is available by phone 24/7 to provide a well-coordinated transfer process and a centralized point of access for all six Ascension St. John hospitals in Oklahoma. With just one phone call to our Transfer Center, you can transfer your patient to the following Ascension hospitals with fewer delays:
- Broken Arrow
- Jane Phillips in Bartlesville
- Medical Center in Tulsa
- Nowata
- Owasso
- Sapulpa
After placing your phone call, an experienced Transfer Center RN will handle the complete transfer process. Our goal is to ensure the continuity of care for your patient. If there is a delay, we will keep you informed every step of the way.
Please be ready to provide the following information when calling:
- Your name
- Referring physician and hospital
- Your phone number
- Service requested
- Level of care needed
- Patient name
- Basic patient condition
How to transfer a patient with one call
- Call is placed to the Ascension St. John Transfer Center toll free number 1-877-774-0013.
- Basic information received regarding patients needing to transfer and requested service(s) are determined.
- Acceptance is provided by a Transfer Center RN on behalf of the accepting Provider.
- Transfer Center RN will direct the sending facility to begin setting up transport
- The on-call provider is paged while bed placement is determined.
- Sending provider is connected with the receiving provider for the report.
- Level of care is confirmed and a patient's bed assignment is provided.
- All other pertinent parties are notified of incoming transfer.
- The sending RN will be connected to receiving RN for the report.
- ETA updates provided as requested.
