Insurance coverage for cardiac rehabilitation

To participate, you will need a Doctor's Referral Form, which we can help you obtain. Once you have a doctor’s order, you can set up an appointment for an initial evaluation. We will take a Medical Assessment and give you more information about the program. Be sure to bring along a list of current medications, including dosage and frequency, along with a copy of any other healthcare instructions.

If you are experiencing chest pain, especially in combination with jaw or arm pain, lightheadedness, nausea, vomiting or difficulty breathing, you might be having a heart attack. Call 911 or go to the nearest emergency room right away.
