As Ascension’s dedicated pharmacy, Ascension Rx provides greater access, convenience and care coordination of your prescriptions through our community pharmacy locations, home delivery and specialty pharmacy options. Ascension Rx pharmacists and technicians create a personalized experience for you and your family while receiving medications using advanced automation to fill prescriptions faster, leaving more time for prescription consulting. How to get started

  • Step 1: Create an account

    Provide us with your contact information and medication allergies by filling out this form for home delivery of maintenance and specialty medications. After filling out the form, we’ll send you an email confirmation and call you to finalize your medication transfer.

    *Patients do not need to create an account for medications that are being filled at an Ascension Rx community pharmacy location.

  • Step 2: Talk to your doctor

    When sending prescriptions to Ascension Rx, doctors should order a 90 day supply of your maintenance medication and a 30 day supply for specialty medications. Ask your doctor to send your prescriptions to:

    • Ascension Rx 1001 Home Delivery for maintenance medications
    • Ascension RX 1303 - Specialty for specialty medications

    Your doctor can also fax prescriptions to 1-833-347-0798.

  • Step 3: Confirmation and payment details

    The Ascension Rx team will call to confirm your prescription information. We will be calling from 1-833-633-7279 (or add Ascension Rx to your contacts).

    To enroll in home delivery, you will need to provide your payment details by filling out this form.

  • Step 4: Medication delivery or pick-up

    Medications are conveniently delivered to you when you choose home delivery. Talk to your doctor about setting up your mail delivery.

If you are interested in setting up your maintenance medications for automatic refills, please call 1-833-633-7279 between 8 a.m. and 5 p.m. CT to enroll. Please note, specialty and controlled medications are not eligible for automatic refills.

  • Charitable medication programs, such as Dispensary of Hope or the Ascension Rx Prescription Assistance Card, that may provide certain medications at no cost through participating Ascension community pharmacies
  • Prescription savings and discount options, including the Ascension Rx Drug Discount Card, to help reduce out-of-pocket costs
