Volunteer application form
Once you complete the application form, you will receive a confirmation email with additional forms to complete relating to confidentiality, your health assessment and background checks.
After we receive your application and required forms and check your references, you will be contacted by our Volunteer Services team.
What to expect next
- Interview with Volunteer Services
- Discover your areas of interest and possible assignments
- A criminal and caregiver background check will be completed
- Meet with an associate health nurse for a health check:
- A seasonal influenza vaccination (flu shot)
- A drug screen
- A blood draw to test for TB and for immunities to measles, mumps, rubella (MMR), varicella (chicken pox), tetanus, diphtheria and pertussis (Tdap)
Complete Ascension Calumet orientation
After your interview, you will be asked to complete an online orientation. You may also choose to complete orientation by watching the following video, reading paper documents and completing a quiz, which will be given to you at the time of your interview.
