Your care team delivers advanced rehabilitation care and comprehensive medical management. We also provide onsite medical specialist consultations, diagnostic tests, and medical procedures as part of our hospital-based rehabilitation units.

Our team focuses on improving your emotional and physical health and involves your family or caregiver team for daily support to create a care plan personalized to your specific needs. Your rehabilitation care team may include:

Case managers to explain the details of your stay and outline any lifestyle changes you may expect. They also work with you and your insurance company to certify ongoing rehabilitation and coordinate your discharge.

Medical director is a doctor trained in rehabilitation who will coordinate your overall treatment program and oversee the efforts of the rehabilitation team caring for you.

Rehabilitation nurses provide round-the-clock nursing care and can help answer any questions you and your family may have.

Physical therapists teach you exercises to help improve your balance, strength and mobility. They may also help you learn how to use walkers and canes, braces, artificial limbs, or other mobility devices.

Occupational therapists focus on improving your everyday living skills, including bathing, dressing, grooming, and eating. They may also provide you with training for equipment to help you better perform these activities.

Speech therapists work to improve your ability to communicate, problem-solve and reason, and address any problems with swallowing.

Your rehabilitation care team may also include a dietitian, pharmacist, respiratory therapist and other specialists. Your therapy sessions and activities are posted daily on a schedule board. Your doctor will continue to follow your progress and remain in charge of your medical care.

Types of post-acute care

If you are hospitalized for a serious illness or injury, your doctor may recommend some type of post-acute care after being discharged from the hospital. We help you reach a higher level of functional movement throughout your recovery.

Other post-acute care options may include home health care with nursing, respiratory care and rehabilitation therapy or a skilled nursing home.

When you leave the hospital to recover at home, your care team at Ascension Calumet Hospital can connect you to outpatient physical therapy clinics and cardiac rehabilitation centers to help you continue your recovery with guided therapy and exercises.
