Ascension St. Vincent Joshua Max Simon Primary Care Center provides support services for you and your family, even if you don’t have health insurance. We care about your emotional and physical health. Our social workers and Spiritual Care chaplains are here to listen and help connect you with resources. Health access
Our health access team provides many services and support, including:
- Find a doctor
- Medications at free or reduced cost through the Prescription Assistance Program
- Application for health coverage
- Application for public programs
- Connection to community resources and agencies for assistance, such as utility assistance, food and housing assistance
Interpreter services
We provide interpreting services for those who need language assistance.
Lab testing
The primary care center has an on-site lab for lab testing.
Diagnostic imaging - radiology
The primary care center has on-site X-ray imaging, as prescribed by your doctor.
Pharmacy
Our pharmacy provides support services to those without pharmacy-based insurance coverage. Talk to us about our 340B program to receive patient assistance and medications that are part of your care plan. Our pharmacists are here to answer your questions about medications and provide chronic disease management services.
Social work
Our dedicated social workers are here to help support you and your family. We provide behavioral and mental health assessments. We can also connect you to community resources.
