The transplant process has multiple phases. These phases are all an important part of the transplant process as they help guide and support you every step of the way. Your care team is here to assist you in navigating your personal experience with our program.
Transplant referral
The transplant process starts with you and your nephrologist (kidney doctor) or primary care doctor. Your doctor will send a referral for an evaluation at Ascension Sacred Heart Kidney Transplant.You will receive a call from our dedicated transplant team to schedule you and your support person for a transplant education class.
Consent for evaluation form
In the education packet, you will find consent forms. You should review these forms carefully. These forms will give an overview of what to expect during the transplant evaluation. A transplant coordinator will be available during the initial evaluation to review and answer any questions you or your support person may have about the evaluation.
Pre-transplant evaluation
After you take the education class, you will have an evaluation with all members of your care team. We take the time to discuss all parts of the transplant process and answer your questions. You can also talk with your care team about financial requirements.
There are times when a patient is determined to be at high risk or unsafe to transplant. Your doctor will talk with you if this occurs, and discuss other options for your care.
Testing
We require routine diagnostic tests to be completed as part of your evaluation. Some testing, such as blood work, electrocardiogram (ECG) and a chest x-ray, will typically be performed the day of your appointment. You will receive a letter of requirements outlining the testing we recommend after your initial evaluation which includes routine cancer screening and cardiac, vascular and organ diagnostics.
Medical Review Board and United Network for Organ Sharing (UNOS) Waitlisting
When all of the testing and appointments are completed, your transplant coordinator will present your case to the weekly Medical Review Board. The members of the Board will decide if a transplant is safe for you.
Next steps
If you are UNOS waitlisted, you and your referring provider will be notified of your waitlist status. You may be listed active and able to receive an organ or inactive with instructions. When listed there will be mandatory annual testing and visit requirements.
Your transplant coordinator will provide individualized information about your status at the time of waitlisting and all requirements necessary to maintain active waitlist status. If you are declined as a candidate, you will be notified and your transplant coordinator will help explain the decision and answer your questions.
