My Health Manager

After watching the weight-loss seminar, you may register for My Health Manager. The site helps us to determine your eligibility for bariatric surgery.

Enroll in My Health Manager

You must watch the Surgical Weight-Loss Center video and complete the test before using My Health Manager. Once approved, you can register for your upcoming doctor visit, from anywhere. After registering, you have the option of subscribing to the full My Health Manager suite of products. When you subscribe to My Health Manager, you join an online community with tools to help you manage your own health and wellness.
