Frequently asked questions about referral and the admissions process

  • How can a provider refer a patient for addiction treatment and recovery care?
    1. Have your patient sign a release of information form before they are admitted.
    2. Fax the completed form to our customer service representatives, Ascension Brighton Center for Recovery, Admissions Department, FAX 810-227-2029.

  • Who can be admitted for addiction treatment?

    Our team at Ascension Brighton Center for Recovery provides personalized care to adults (ages 18 and older) experiencing acute intoxication, chronic substance use of one or more substances (alcohol, opioids such as fentanyl, and other types of chemical dependency). Patients must have a primary diagnosis or history of a substance use disorder or substance dependency and be physically and mentally stable.

  • Once admitted, how will I be updated about my patient?

    Once we have a signed release of information, a substance use disorder therapist assigned to your patient manages your his or her treatment plan. This therapist will keep you apprised of your patient’s status after our initial assessment is completed and treatment is initiated. At the time of discharge from primary treatment, we’ll contact you to discuss a coordinated recovery care plan.

    Note: If a patient has not completed a release, or has revoked an existing release, we will not be able to provide any information regarding his or her progress in treatment. Our therapists will, however, make every effort to obtain a release and keep the patient’s healthcare provider informed.

  • How can I help a patient self-refer for treatment?

    Our team at Ascension Brighton Center for Recovery knows the work of addiction recovery does not stop once a patient leaves our campus. If you have a patient who is experiencing a relapse, contact our team at Ascension Brighton Center for Recovery. 

    To self-refer, a patient can submit a secure online admission form for a confidential assessment by our care team, or call our admissions department at 1-877-9-SOBER-1 (877-976-2371)

    Admissions representatives are available:

    • Monday - Friday, 7 a.m. - 5 p.m.
    • Saturday and Sunday, 7 a.m. - 3:30 p.m. 

    After-hours calls: 

    A member of our clinical staff answers calls made outside the hours listed above. Information is logged for prompt attention by an Admissions representative the following business day.
