In-home rehabilitation
Our certified brain injury specialists lead the Ascension Macomb-Oakland Hospital Community Monitory Living Program. Our team provides weekly visits and functional training in the home to support your recovery. We’ll assess your living environment to help you transition from hospital and inpatient rehabilitation to home care. Our program is for individuals living within the tri-county and surrounding areas. Working together, we’ll help you or your loved one relearn and improve many skills, including:
- Community reintegration
- Daily living activities
- Medication management
- Money management
- Organizational skills
- Preparing for an emergency
- Problem solving
Our team also provides respite care – a service thatprovides day care so caregivers in the home can rest, do errands and spend time with other family and friends.
Outpatient therapy for brain injury recovery
At Ascension Macomb-Oakland Hospital we provide comprehensive outpatient rehabilitation therapy. Our team of certified brain injury specialists includes physical therapists, occupational therapists, speech therapists and social workers. Your team works together to create a personalized care plan that supports your brain recovery needs. We provide the latest therapeutic treatments, including:
