If you have congestive heart failure or other heart conditions, our heart care team specializes in anticoagulation medication management. We monitor the blood thinning medications to help prevent blood clots and keep existing clots from getting larger.
C.O.A.G. Clinic
The C.O.A.G Clinic is part of the Ascension St. Vincent Heart Center Medication Management Program. The C.O.A.G team includes pharmacists and registered nurses. Each care team member is trained to monitor the anticoagulation therapy, and report any changes to your doctor. Our heart care eductors provide additional support for ongoing recovery and smoking cessation.
It's our goal to keep your anticoagulation therapy safe and effective. At every appointment (or by phone), we can answer your questions about the medication and any symptoms you may be experiencing. We will also talk to you about medication adjustments. By using easy-to-understand instructions, many of our patients can safely and effectively self-dose and use an at-home monitoring device.
