Part of St. Vincent's Heart Center services, the Ascension St. Vincent Health Medication Management care team at 13500 North Meridian Street includes pharmacists and registered nurses. Each is trained to monitor your drug therapies, including anticoagulation therapy, and report any changes to your physicians. Conveniently located on the campus of Ascension St. Vincent Carmel, we are here to answer your questions about your medications and any symptoms you may be experiencing. Our team members will talk to you about medication adjustments. Using easy-to-understand instructions, many of our patients can safely and effectively self-dose and even use an at-home monitoring device.
