The pre-admission care team at Ascension Saint Joseph - Chicago Pre-Op Testing helps get you ready for surgery by reviewing your medical charts and evaluating your health. Our care team talks to you about your medical history, including any chronic health conditions. You will also receive information about the anesthesia you’ll receive prior to your procedure. We listen to you and your health concerns. Then, we work with you to create a preoperative care plan that’s right for you.
What to expect during your preoperative evaluation
Your care team will do a preoperative evaluation prior to your surgery. We coordinate all aspects of your surgical care, answer your questions and provide dietary instructions. If you are under the age of 18, a parent or guardian must sign the consent forms required for testing and surgery.
What to bring for preoperative testing
When you come to pre-admission testing, please bring:
- All of your medications, including over-the-counter medications and supplements
- Your insurance card for completing your admission form
- Any doctor’s orders or records you’ve received
- A copy of your living will and/or durable power of attorney for your medical record (optional)
- Guardianship papers, if appropriate.
