Getting connected to your medical care

You have two ways to access medical information online at Ascension Michigan locations in southeast Michigan.
Doctor's office portal

  • View test results
  • To sign up for a portal for your doctor office visits, call your doctor's office or simply talk to them when you are there for a visit.
Hospital portal
  • View tests you received at a hospital (labs, imaging, procedures)
  • Visit summaries
  • To sign up for a portal for your hospital visits, go to any registration desk in your local Ascension hospital. After providing your basic information, you will receive an email with an activation link. Follow the prompts to complete the sign-up process.

Most of your hospital records can be downloaded and printed from your online patient portal. You can also directly request this information online using the button below for hospital locations in Detroit, Warren, Madison Heights, Southfield, Novi, or St. Clair.

Doctor's office medical records

To receive a copy of your medical records from one of our clinic locations, please contact the clinic directly for further instructions. If you need to look up the contact information for the clinic, you can search our locations.

Frequently asked questions

  • In most states, you must be 18 years of age or older to sign up for a patient portal. Some states allow patient portal access at age 13. Please talk with your child's provider to discuss the options for patient portal access for your child in your state.
  • Laws vary by state. In some states, you may sign up for access to your child’s information if they are under 13. Once they reach age 13, a child can give “proxy” access to another adult. Once a person turns 18, they need to register for their own account by talking to their healthcare provider. Other states do not allow parents to sign up for a portal for their child. Please talk with your child's provider to discuss the options for patient portal access for your child in your state.
  • Like your paper-based record, your electronic health record is protected by the Health Insurance Portability and Accountability Act (HIPAA). Your privacy is very important to us and your health information is always kept confidential.
  • This isn’t recommended. Each person should have his or her own account. That way, you will always have the choice in the future to stop sharing your information with another person.
