Open since 2013, the Ascension Alexian Brothers Hospice Residence is a 19,150 square-foot, freestanding hospice house in Elk Grove Village. Our spacious, home-like residence has 16 private rooms with windows overlooking landscaped gardens. Our care team supports patients who require long-term care and those with serious symptoms, such as pain or shortness of breath.
How to arrange for hospice care
If you are seeking hospice care, start a conversation with our care team and take a tour. Our billing specialist can answer your questions about insurance coverage and payment options. Your loved one must qualify for hospice to reside here. We are licensed to take care of both general inpatient (GIP) and routine level of care patients. At The Ascension Alexian Brothers Hospice Residence, any days that a patient meets criteria for the GIP level of care will be covered by Medicare (if the patient is Medicare eligible). Hospice services may also be covered by Medicaid and other private health insurance.
Levels of hospice care
There are four levels of hospice care available, depending on your and your family’s needs. During your time at Ascension Alexian Brothers Hospice Residence, you may receive several levels of care. Your care team monitors you and your health needs before recommending a different level of hospice care. Types of hospice care include:
Routine hospice care: The patient receives hospice care at their current residence, wherever that may be — home, assisted living center or a nursing facility. This is the most common level of hospice care.
General inpatient care: If the patient’s symptoms progress and care can not be provided in the “home,” 24/7 general inpatient care can be provided in a Medicare-certified hospital, hospice inpatient facility or nursing facility with registered nurses.
Continuous care: This type of care is provided during times of crisis when the patient is at risk of hospitalization. Our skilled nursing team provides care to the patient for longer periods — 8 to 24 hours a day.
Respite care: Our care team provides temporary relief to the patient’s primary caregivers. The patient is typically being cared for in a long-term care facility with 24-hour nursing availability. Respite care can be provided for a maximum of five days per benefit period.
