If you have not already been contacted by our clinic to transfer your care to another physician, please contact our office and our clinic staff will assist you in setting up continued care through another AMG physician at your current location.

Your medical records will remain at your current clinic location unless otherwise directed. If you would like to arrange the transfer of your records to another physician outside of AMG or obtain a copy of your patient records, please contact our office, and we will work with you to transfer your information as requested.
