Now is the time to take charge of your health and make a change.
The diabetes prevention program is a year-long program which focuses on lasting lifestyle changes like regular physical activity, healthy eating, weight loss, and stress management. The program meets for 16 weekly sessions and then 6 monthly follow-up sessions. This program helps you make lifestyle changes to reduce your risk for type 2 diabetes. You must have risk factors or a diagnosis of prediabetes to meet the class requirements. Pre-diabetes can be diagnosed by your physician or you can take the self-screening test by going to the following Web site: Do I have prediabetes
If you have a diagnosis of type 2 diabetes you will not qualify to attend and we recommend our Conversations in Diabetes.
