Diabetes Prevention Program Ascension MI

Join us for a FREE group-based, structured lifestyle change program for people who are at risk or have prediabetes.

Now is the time to take charge of your health and make a change.

The diabetes prevention program is a year-long program which focuses on lasting lifestyle changes like regular physical activity, healthy eating, weight loss, and stress management. The program meets for 16 weekly sessions and then 6 monthly follow-up sessions. This program helps you make lifestyle changes to reduce your risk for type 2 diabetes. You must have risk factors or a diagnosis of prediabetes to meet the class requirements. If you have a diagnosis of type 2 diabetes you will not qualify to attend and we recommend our Conversations in Diabetes.

We have different choices, virtual or in-person.

Registration is required. Program enrollment is limited. Informational session (first) is mandatory. These are a series of workshops built on the previous session. No new admittance after the third week. Workshops are FREE.

Participants will be contacted before the class date from the Lifestyle Coach Facilitator to qualify for this class.  

If your class required payment, a refund will be given if a cancellation request is received 2 business days (48 hours) prior to the start of the class/program. A full refund will be given for any class cancelled by the site.
