Poclain Personal Health Risk Assessments 2022

Welcome to the Poclain employee and spouse Health Risk Assessments registration.

Your screening will include the following: blood pressure, height, weight, body fat %, hip/waist measurement, BMI, a fasting blood draw (lipid panel and glucose), A1C for over age 40, PSA for men over age 50, and cotinine test. You must fast 10-12 hours before your appointment. Avoid all foods, snacks, and beverages; water is acceptable. Please drink plenty of water before screening. Consult with your physician if you have questions about fasting and your medications. Avoid all alcohol and caffeine prior to the screening.

Please wear a top that is short-sleeved, sleeveless or easy to roll up over the elbow for the blood pressure screening and blood draw.

There are NEW guidelines for you to review prior to arriving to your scheduled appointment:

  • Do not arrive early – please come at your appointed time slot

  • No walk-in appointments available

  • Please wear a mask upon arrival and throughout the screening

  • After each appointment the areas will cleaned with disinfectant spray and/or wipes

  • Please maintain a 6 foot distance between individuals

If you are experiencing any COVID-19 symptoms below, please reschedule your appointment.

  • Fever of 100.4 degrees or higher when taken by mouth

  • Muscle pain or chills

  • Runny nose/congestion (for chronic allergies, a change in baseline)

  • New loss of taste or smell

  • Nausea, vomiting or abdominal pain

  • New onset of severe headache, especially with a fever

  • New uncontrolled cough (for chronic allergic/asthmatic cough, a change in baseline)

  • Sore throat

  • New unexplained rash
