Fox Valley Tool and Die Health Coaching Plant 2 2020

This coaching program is for any Fox Valley Tool & Die employee or spouse that participated in the health screening through the wellness program that would like to discuss their biometric values or other health and wellness topics. If you did not receive a score of 75% or higher, health coaching sessions are required to be completed in order to be eligible for the wellness discount. The number of coaching sessions you are required to complete depends on your score. The deadline to complete the required coaching sessions is August 31, 2020
