Consent for Treatment of a Minor Child

Use this form as you travel and make provisions for someone to care for any of your children not going with you. This consent form and medical data questionnaire will be valuable should your child be ill or injured while you are away.

Safe Sitter

What it is: A medically accurate program that teaches girls and boys ages 11 to 13 how to handle emergencies when caring for younger children.

The goal of Safe Sitter is to reduce the number of avoidable and unintentional deaths among children being cared for by babysitters. To successfully complete the Safe Sitter program, the students must attend a one-day class and pass a practical and written test to show that they have mastered the key concepts and have the skills necessary to handle an emergency.

Diabetes Management and Education Classes

Diabetes management and education classes are offered on a monthly basis by the registered dietitian and registered nurse specializing in diabetes. Classes are held in the Sr. Andrea room on Mondays. For more information call 765-552-4687.
