How to schedule a mammogram

To schedule a mammogram, you will need the following: your email address, health insurance, referring doctor (if you don’t have one please leave the section blank), and your basic health information. A referral is not required for a screening mammogram.

Step 1

Select "schedule a mammogram" below. Next, choose your appointment based on “Soonest available” or “Closest to me” and confirm the location.

Step 2

Select “Screening” as your appointment type. Use the arrows to scroll through calendar dates or select the next available date for a screening.

Step 3

Answer a few simple questions about your health then complete the “Patient and Visit Information” form. Upon pressing “Submit,” you’ll be registered and an appointment confirmation will be sent to your email.
