Who will help me after I am discharged from the hospital? Ascension hospital care includes care coordinators who help you get the transitional care you need after being hospitalized. It’s called Post-Acute Care (or subacute care) services. We know it can be stressful to leave the hospital and wonder how you will do it all on your own. You don’t have to. Our Care Coordinators will explain how your primary care healthcare team will be monitoring you as you heal. With your physician’s recommendation, we’ll also identify the best place for you to continue the healing process. Post-acute care services include:

  • Home Health Care
  • Hospice
  • Long-term Acute Care Hospitalization
  • Skilled Nursing Units
