The Jane Phillps Diabetes Program care team of certified diabetes educators are committed to providing individual, goal-centered education, and will work with you and your doctor to develop a program for education and training to manage your diabetes.

The Diabetes Program is accredited by the American Diabetes Association and recognized as meeting the national standards for Diabetes Self-Management Education.

Services offered

The Diabetes Program offers both inpatient and outpatient services for people with diabetes or pre-diabetes.


If you are admitted to the hospital for an illness or surgery, we know that your diabetes can be affected and can slow your recovery.  Our diabetes team will make sure you get the coordinated care you need while in the hospital.


Our outpatient services will give you the information and support you need to control your diabetes:

  • Diabetes management classes
  • Meal planning
  • Blood glucose monitoring
  • Insulin instruction
  • Insulin pump and continuous glucose monitor training
  • Diabetes during pregnancy
  • CDC National Diabetes Prevention Program
  • Personal sessions for special needs

A physician referral by your primary care provider is required.  Call your primary care provider to request the required physician referral.

Medicare and many insurance providers pay for diabetes education. Some plans require a co-payment at every visit.  We recommend you check with your insurance provider to determine exactly what is covered under your individual plan.
