Ascension St. Vincent's heart failure specialists start by listening to you and your heart. We specialize in diagnosing and treating heart failure. If you are diagnosed with heart failure, we help you transition from the hospital to your home. After being discharged from the hospital, we connect you with the Cardiac Transition of Care Clinic . Our clinic will help you manage your heart condition until you are seen by your primary cardiologist.

Our heart failure program is recognized by the American Heart Association as a Get With the Guidelines-Heart Failure GOLD PLUS with Honor Roll award. This recognition is granted to programs that apply the most up-to-date and evidence-based treatment guidelines to improve patient care and outcomes.

At Ascension St. Vincent, we perform nearly 40,000 cardiovascular tests and procedures each year. These non-invasive tests can detect heart function issues before they become more serious. Our cardiologists specialize in managing all stages of heart failure.

Diagnostic cardiac tests include:

  • Cardiac nuclear imaging
  • Echocardiogram (TTE)

Ascension St. Vincent's Cardiology is your partner in heart disease. Our Cardiac Transitions of Care Clinic is a bridge from hospital care to home. We have expertise in treating all stages of heart failure and focus on helping you manage your condition. Our goal is to provide you with information on how to take care of your heart to avoid a more serious condition. In our transition of care clinic, you will see the pharmacist and nurse practitioner to assist you in your transition from the hospital to home. Medications and management of heart failure will be discussed in detail.

If you are hospitalized due to a heart attack that leads to heart failure, our team monitors your progress in the hospital. After discharge, we help transition your care to our outpatient clinic. At your first appointment, you'll learn more about your care plan, including ways to improve healing and better heart health.

Heart Failure University In addition to the Heart Failure Clinic, we offer a program at no cost to you about ways to manage heart failure in all aspects of your life. By participating in eight classes over four weeks, you'll learn about:

  • Anatomy and heart function
  • Heart failure medications
  • Advanced care planning
  • Nutrition
  • Stress management
  • Treatment options
  • Physical and occupational therapy (exercise recommendations)
  • Lifestyle changes
