Access Your Health Information

Ascension CSM and Madison Medical Affiliates make accessing your health information from primary care visits, hospitalizations, and ED visits secure and easy to use with the latest technologies.

By enrolling in CSMConnect and our online services, you can:

  • Access your health information from your electronic medical record.
  • Request prescription renewals and appointments.
  • Review laboratory and radiology results.
  • Send secure messages to your health care provider.
  • Access patient education materials.
  • Schedule, view and cancel your appointments.
  • Pay your bills online.

Enroll in CSMConnect

Option 1 (In-Person): You can enroll in person by speaking with your provider’s office staff or the hospital’s registration staff. You will be asked to verify your identity. You will receive an email invitation to join the portal. Follow the instructions in the email to complete the registration process.

With CSMConnect, you can gain access to the records of your younger children, and with permission, the records of other adults for whom you are a caregiver. Speak with your office or hospital registration staff for details. (Due to Wisconsin State Law, the records for children between the ages of 12 and 17 cannot be accessed by a parent or guardian.)

Option 2 (Self Enroll): To self-enroll, click here and follow the instructions on the registration page. To verify your identity you will be asked to enter your Medical Record Number (MRN) or an email address already on file. Click here for instructions to help you find your MRN. Note: You must be at least 18 years of age to register using this method.

Log Into CSMConnect

Once your account has been set up, you can access it anywhere from the internet (e.g., at home, work, school, library) by visiting CSMConnect link.

CSMConnect is also accessible using the HealtheLife App which can be downloaded from the App Store for use on your iPhone or Android.
