The Ascension Saint Thomas Inpatient Rehabilitation unit is designed with “real-life” therapy areas - like a working kitchen and a simulated automobile - to prepare patients for returning home.
In preparation for discharge, the patient and family are trained by the rehabilitation staff in the skills needed at home. In order to ensure a safe transition home, family members who will be helping in the care of the patient will attend therapy sessions to learn how to provide assistance in the home setting. Our staff will provide individualized written discharge instructions that patients and their family caregivers can refer to once they return home. We will also arrange for any services or equipment that the patient may need after discharge.
