Ascension St. Vincent retains hospital-based medical records in accordance with Federal and Indiana laws and regulations. Information contained in the patient’s medical record is confidential. For this reason, all requests for medical records must include a signed authorization by the patient (see the Medical Records Authorization form under locations).
Ascension St. Vincent HIM – Medical Records Department securely maintains electronic copies of our Ascension St. Vincent patients’ medical records as they relate to inpatient and outpatient services for hospital, outlying surgery center procedures, radiology and therapy services.
A fee is charged to cover the processing of requests. However, a fee is not charged when the copies requested are to be sent to a physician, as those requests are a part of your continuing care.
