Join us for an In Person group-based, structured lifestyle change program for people who are at risk or have pre-diabetes. The diabetes prevention program is a year-long program which focuses on lasting lifestyle changes like regular physical activity, healthy eating, weight loss, and stress management. The program meets for 16 weekly sessions and then 6 monthly follow-up sessions. This program helps you make lifestyle changes to reduce your risk for type 2 diabetes. You must have risk factors or a diagnosis of prediabetes to meet the class requirements. Pre-diabetes can be diagnosed by your physician or you can take the self-screening test by going to the following Web site: doihaveprediabetes.org. If you have a diagnosis of type 2 diabetes you will not qualify to attend.

Registration is required. Workshop size is limited. Informational session (first) is mandatory. These are a series of workshops built on the previous session. No new admittance after the third week. Workshops are FREE.

Participants will receive a telephone call before the class date from the Lifestyle Coach Facilitator to qualify for this class.  Once you qualify, you will receive an email with a link to the class. 

If your class required payment, a refund will be given if a cancellation request is received 2 business days (48 hours) prior to the start of the class/program. A full refund will be given for any class cancelled by the site.
