One in three adults have prediabetes. Ninety percent of adults with prediabetes do not know that they have prediabetes.  Lifestyle changes can prevent or delay type 2 diabetes and other health conditions.

The diagnosis of prediabetes means that although your blood sugar is higher than normal, it is not high enough to be diagnosed as diabetes. People with prediabetes are at significantly increased risk of developing diabetes in the next 5 years. Fortunately, the risk of developing diabetes can be significantly decreased with appropriate lifestyle changes. Your provider may also recommend treatment with certain medications.

The Lourdes Diabetes Prevention Project staff will work with you and your primary care provider to help you make the changes needed to decrease the risk of developing diabetes:

  • Connection to community resources
  • Connecting you to support groups; walking groups
  • Access to a health coach for one-on-one advice for making lifestyle changes
  • Referrals to specialty services, when needed
  • Be available to answer questions that may come up
  • Monitor your progress and provide regular reports to you and your primary care provider
  • Provide information about pre-diabetes and strategies that may help you make the lifestyle changes needed to decrease the risk of developing diabetes.
  • Help setting goals for your health
  • Special membership pricing to Synergy Athletics to encourage exercise
