At Ascension Saint Agnes, our Anticoagulation Clinic provides compassionate, personalized heart care. Being on blood-thinning medications can be stressful and overwhelming. Warfarin (Coumadin®) is a blood thinner that is used to prevent blood clots from forming in the body. Our nursing-directed care team is dedicated to helping you maintain a healthy international normalized ratio (INR) and adapt to your customized daily care plan.

When you come to the Saint Agnes Anticoagulation Clinic located at the West entrance on the first floor of the hospital for a finger-stick test, the test results are immediate. Dosage adjustments are made onsite at the time of the test. The test determines how long it takes for your blood to clot. Since everyone responds differently to Warfarin, more frequent visits may be needed to determine the correct dosage of Warfarin for you. Once the dosage has been stabilized, visits will occur every four to six weeks.

We offer convenient hours by appointment only, Monday-Friday from 7:00 a.m. – 5:00 p.m. Parking is available at no charge and $5 valet parking is available if preferred.

Regularly scheduled appointments are important for your safety and ongoing medical care. It is extremely important that you keep your scheduled appointment. Follow-up appointments are scheduled based on national anticoagulation standards.

At every appointment bring your health insurance card and photo identification and a list of all medications you are taking, including over-the-counter medications, vitamins, herbals and supplements. Our nurse will ask you for your health and medical history, including your exercise and eating habits.

The Saint Agnes Anticoagulation Clinic provides immediate results and dosing recommendations with a printed dosing instruction sheet and assistance with prescription refills.
