Medical Records Request

As a patient, you have the right to access your medical records. Here you'll find instructions and a convenient form to help us process your request.

To request copies of your medical records, please print and fill out the Authorization to View/Disclose Health Information. Once you have completed this form, you may:

Drop it Off - Authorization to View/Disclose Health Information forms are accepted during business hours. Please bring a photo ID when dropping off this form.

Mail - Authorization forms may be mailed to the address of the facility you select below. Please include a copy of a photo ID or include a notarized signature.

We are unable to accept authorizations via e-mail due to state and federal laws.

Requests are usually completed within one week but may be delayed if your medical record is not available. Requests may be completed in less than one week if the request is for a medical need.

A photo ID is required when picking up copies of medical records.

Viewing Your Medical Records

Accessing your records using My Chart is a quick and convenient way to view your medical records. Please see the Ascension Website on how to obtain a My Chart account. If you prefer to view records in person, you will need to make an appointment at the specific hospital or clinic. Please call the site-specific number listed below. Appointments are scheduled as soon as possible.

Copy costs may vary, please select a facility below for contact information to obtain current copy rates or to download an AUTHORIZATION TO VIEW/DISCLOSE HEALTH INFORMATION form, and view instructions on how to complete it.

Care Everywhere®

Ascension Wheaton Franciscan Healthcare, part of Ascension, discloses your health information without your written permission when other hospitals, physicians, and health care providers need to treat you. The exchanging and sharing of your health information between the physicians who treat you is helpful in the continuity of your care. Your Ascension Wheaton health information is automatically included in this electronic exchange through Epic’s Care Everywhere® unless you request in writing for it to be excluded.

To have your health information excluded from Care Everywhere®, please complete the Care Everywhere® Opt Out Form and submit to the facility that created your health information.
