What to Expect: First Visit
To expedite your first appointment, please arrive 15 minutes early to complete registration forms so that we have all the necessary information to treat your child.
It is helpful for our doctors to have records from your previous doctor to review prior to your first visit if possible. We can sometimes obtain immunization dates from the Wisconsin Immunization Registry. Since not all offices update the registry in a timely fashion or if you have moved from out of state, it is helpful to obtain records from your previous provider. You can complete a medical records request directly from your previous doctor or use our form below to request those records be released to us.
Your children's records are held in strictest confidence and cannot be released without your written consent. This is to protect your children's personal medical information from unauthorized inquiry. Immunization records will not be given over the phone. We can fax small amounts of information (such as daycare forms, immunization sheet, and physical exam forms) if a written release is on file.
Emergencies and After-hours Care
If your child is experiencing a life-threatening emergency, call 911. The hospital ER will contact your pediatrician. Otherwise, our answering service receives after-hours calls and contacts the on call pediatrician regarding urgent care and medical emergencies. The doctor will then contact you to discuss next steps.
