IMPORTANT NOTICE: THIS ONLINE PRE-REGISTRATION PROCESS WILL NOT SCHEDULE YOUR APPOINTMENT WHERE YOUR PROCEDURE IS BEING PERFORMED. YOU MUST FIRST CALL AND ARRANGE YOUR APPOINTMENT PRIOR TO COMPLETING THIS PRE-REGISTRATION PROCESS.

What you will need to pre-register:
- Please submit this online information at least three business days prior to your scheduled visit, or three weeks prior to your expected delivery date. This allows us adequate time to verify and pre-certify your insurance (if applicable)
- By pre-registering you will be able to report directly to your service area. You will not need to visit our Patient Registration area.
Before you begin the pre-registration process, you will need:
- Your insurance ID card information
- Procedure date and time
- Type of procedure (such as a mammogram or X-ray)
- Personal contact information
- Physician order
After you have finished online pre-registration
After you have submitted your online pre-registration form, you will receive a confirmation e-mail from the hospital. You will also be reminded to print your completed form and bring it with you on the day of your procedure, along with any items listed on your pre-registration form.
